Forms and Releases
The below documents need to be returned to Dr. Hall in person or mailed to ILI Travel at P.O. Box 552, Doylestown, PA 18901. Please do not email the papers as some will be notarized, and we will need the originals.
Travelers Under 18
- For the travel waiver form, please have it notarized.
- For the Guardianship form, it needs to be completed only for those who are under 18 before the date of departure. Please have it notarized.
- Photo release should be completed for all. It does not need to be notarized.
